Please, fill in the following form to request your registration. Once Achilles has received the form and confirms the payment, we will send you SICLAR questionnaire). Keep in mind that our mail will be sent to the address specified in this form.

All the information provided to Achilles will be listed on SICLAR database and will be available only to all authorised users of this database (Subscribing Companies).

The registration is valid for fourteen months and starts two months after the Registration Department send you the access code. The first two months are not counted against your 12 months because it is considered administrative time and use to insure your Company's proper registration. If the file is approved before the two months period foreseen, you will have more than one year of registration. If you cannot complete your Questionnaire during these two months, we will still process your file, but be advised your registration start date remains the same.

From the moment you receive the receipt , you have 30 calendar days to submit the required documentation. Even if you don’t fulfill this requirement, Achilles will retain the fee previously paid, due to the administration and follow up work done.
